Specimen will be rejected if overt hemolysis.
Type and Screen specimens for possible crossmatch are acceptable when collected by a Sanford Lab that has performed annual patient identification and specimen labeling exercises.

Special Instructions
If the patient is an outpatient of hospital patient without a hospital identification armband or a patient being discharged and returning from surgery, the phlebotomist will place blood bank identification (BBID) armband on the patient and a numbered sticker from the BBID band is placed on the specimen and request form. Extra numbered stickers are sent to Transfusion Services.
Transfusion or pregnancy history in the past 3 months should be noted when drawing outpatient pretransfusion T&S specimens
Pre-surgical T&S specimen can be drawn up to 30 days before surgery if patient has not been transfused or pregnant in the past 3 months and blood bank identification red armband is given to patient.
